The US Food and Drug Administration (FDA) in July 2019 requested Allergan, the maker of a macro-textured BIOCELL breast implant and tissue expanders (which stretch a woman’s skin prior to breast reconstruction), recall them from the US market due to the risk of breast implant-associated anaplastic large cell lymphoma (BIA-ALCL), a rare, treatable but potentially deadly cancer of the immune system linked to textured breast implants. The company complied with the request, reports Self. There have been similar recalls in other countries, including France, Canada and Australia. The previous February the agency announced an increase in the number of women in the US diagnosed with BIA-ALCL, according to WebMD.

The FDA gave its first warning of risks from these implants in 2011 when there were 60 cases worldwide. The first case of BIA-ALCL was reported in 1997. There were 573 known cases globally and 33 related patient deaths from BIA-ALCL as of August 2019. Of the 573 cases, 481 of them (or 84 percent of the cases where the implant manufacturer is known) occurred in women with the BIOCELL implants.

What makes these implants different?

Macro-textured breast implants have larger, deeper divots on the outside compared to a micro-textured or smooth implant. Textured implants are only about 10 percent of the implants sold in the US but are popular in other countries. Fewer than half of the textured implants used in the US are macro-textured, including the recalled BIOCELL model. Their texture may help the implant stay in place and reduce the risk of a complication called capsular contracture, which happens if the scar capsule that forms around an implant during the healing process begins to tighten, squeezing the implant.

What is breast implant-associated anaplastic large cell lymphoma?

BIA-ALCL is not breast cancer. It’s a type of non-Hodgkin’s lymphoma that develops around the implant in the fibrous scar capsule that forms around the implant or in the fluids inside the capsule. It normally shows itself through localized symptoms that start years after the implant surgery. The median time for diagnosis is eight years from the time of the procedure.>

The main symptoms are persistent swelling or pain in the area of the implant or a lump under the skin. Swelling, caused by fluid around the implant, happens soon after the cancer develops, causing a noticeable asymmetry between the breasts. Treatment is normally surgical removal of the implants and surrounding scar tissue. The cancer generally grows slowly and doesn’t spread. In some advanced BIA-ALCL cases it can metastasize to nearby lymph nodes then throughout the body. Surgery is usually enough to treat most cases, especially if it’s diagnosed in an early stage but some women with advanced an advanced case might need chemotherapy or radiation therapy to get into remission.

What causes BIA-ALCL is unknown. Some women seem to have genetic mutations making them more prone to develop it and the longer the implants are in the body, the more likely the cancer will occur. Another factor may be BIOCELL implants shed their contents at a higher rate than other kinds of textured implants, which may trigger an inflammatory response near the implant. Inflammation is normally a natural and harmless immune response, but this local inflammation may, over time, cause chronic inflammation resulting in DNA damage in cells which causes them to mutate and that leads to the development of the BIA-ALCL.
